In order to fix your disabled ipod touch 4g due to many attempts of entering many codes is you must restore it to its original iOS or higher. If you have an iOS firmware lower that the expected latest iOS version and restore it, you will expect an error, “3194″ error, which means that you must restore your Apple Device to the latest version, which is 6.1.5. So, first thing to do is to download the latest version of Ipod Touch 4G ipsw (iPod4,1_6.1.5_10B400_Restore). You too must have the latest Itunes software installed on your computer, and an Apple USB cable 
How To restore Ipod Touch 4G To iOS 6.1.5
1. Open Itunes. Insert USB cable on your Ipod Touch. Notice. The Ipod Touch doesn’t detect by the Itunes software. Why? Simple because is was disabled. What;s the solution? You must put the Apple Device into Recovery Mode. How? Long press both power on and home button for about 10 seconds. WHile holding home button key, unpress the power button key for about 15 seconds. Notice that on your PC, it will install the Ipod Touch drivers. Just install it and it must be successful. Then Itunes software will detect that an Ipod Touch was in Recovery Mode.
2. Now that the Itunes detect the device, on your PC press “shift key”, and on the itunes software press “restore” and browse on the ipsw or iPod4,1_6.1.5_10B400_Restore.
